Upcoming Vaccination Opportunity at Madison
Posted on 08/13/2021
The City of Appleton Public Health Department will be conducting COVID-19 vaccination clinics at Einstein, Kaleidoscope, and Madison Middle Schools next week during each site’s on-site registration opportunities. The Pfizer-BioNTech Covid vaccine will be administered.

 - Monday, August 16, 2021, at Kaleidoscope Academy (318 E. Brewster St.) from 2:00 p.m. - 6:00 p.m.
 - Tuesday, August 17, 2021, at Einstein Middle School (324 E. Florida Ave.) from 8:00 a.m. - 1:00 p.m. and 3:00 p.m. -  7:00 p.m.
 - Thursday, August 19, 2021, at Madison Middle School (2020 S. Carpenter St.) from 2:00 p.m. - 6:00 p.m.

There is no need to pre-register. When you arrive at the school, please follow the signage to the vaccine clinic. These clinics are open to anyone 12 and above. Your student does not need to attend the school to receive the vaccine during the clinic at a particular site. Students under 18 must have a parent/guardian with them for the vaccine administration. Parent/guardian consent for minors is required. Consent forms will be shared on-site during the vaccination clinic.

Vaccination is the best and most essential tool for ending this pandemic. We will continue to promote and encourage vaccination opportunities for eligible populations.

If you have any questions or need more information please call the City of Appleton Public Health Department at (920) 832-6429.
